Medical Homes DC
MHDC was created out of DCPCA’s determination to make sure that every District resident has access to quality primary health care – regardless of their ability to pay. We believe that Medical Homes DC is a compassionate approach to get our neighbors who lack adequate health insurance exactly what they need – access to primary health care, when and where they need it. We aspire for everyone in our community to be covered and cared for, and uphold that anyone can become a partner and help with this mission. Capital ProjectsDCPCA developed Capital Projects – an approximately $21 million grant program that leverages dollars from the DC government to assist individual community-based health centers. DCPCA’s current goal is 13 projects, which will expand capacity by about 200,000 additional patient visits – enough to meet the primary care needs of DC’s low-income residents for the foreseeable future. Health Information TechnologiesDCPCA seeks to strengthen the DC public health system by ensuring that health care providers have better access to clinical information in primary care settings, as well as during emergencies. Health Access SolutionsAdolescent Wellness InitiativeAs Adolescent Ambassadors, teens learn about their own health and well-being from leading professionals – in health, law, finance, communications, business, and etiquette – and speak up and promote health and wellness throughout their community. Community Health WorkersA CHW Workforce Development Program will not only strengthen our health care workforce; it will improve access to services, enhance patient compliance, upgrade the quality and cultural competence of service delivery, and reduce the cost of care. Emergency Room Diversion Pilot ProjectDespite having some of the most progressive health insurance coverage in the nation, the highest number of per capita hospitals and doctors, and a growing safety net, many DC residents still turn to the emergency room for their primary health care.
