Million Hearts Learning Collaborative Webinar: "Making a Difference with Diabetes"

Webinar Overview:   In this one (1) hour interactive webinar, we will discuss a new lens for how behavioral health providers (BHPs) can make a difference in regards to the management of diabetes in primary care.  Specifically, we will discuss evidenced informed interventions that BHPs can use to provide direct patient care.  Additionally, we will discuss how BHPs can work as part of the medical team to help ensure interprofessional collaboration and bidirectional upskilling of team members.  Lastly, we will discuss how BHPs can help support the entire primary care system with regards to management of diabetes.  We will use a case study to illustrate these as well as field questions from the audience.
