Overview
For over 40 years, Community of Hope has worked to improve health and end family homelessness to make Washington, DC more equitable. We believe that everyone in DC should have good health, a stable home, family-sustaining income, and hope.
1) Improve health and eliminate inequities in health outcomes in under-resourced communities in Washington, DC.
2) End homelessness for families in Washington, DC.
3) Innovate, collaborate, and integrate to improve well-being for families and communities.
4) Be a strong and sustainable organization, ensuring we have the people, resources, and expertise needed to advance our work.
Major Achievements
In the past 10 years, Community of Hope has sustainably grown to over 4x over. This period has seen expansive growth for our health services. We have opened a new health center in Ward 8, renovated and expanded our Ward 1 center, relocated and tripled the size of our Ward 5 center, and added a community resource hub. We serve over 14,000 patients a year, providing medical, dental, emotional wellness, and patient care coordination.
In 2022, we were recognized for being in the top 25% of community health centers for our health outcomes in eight areas. We also earned four distinctions, including in the areas of enhancing access and reducing health disparities.
Our results are a testament to our compassion, expertise, and commitment to innovation. We offer maternal fetal medicine specialty services, sexual health services, team-based care for chronic disease patients, and WIC at several of our sites. Since 2014, we have been recognized six times as a Washington Post Top Workplace, based off the feedback of our 400-member team.
